If your cookies are too brown
Was the cookie baked too long, or was the oven temperature too high?  Use a
stand-alone oven-safe thermometer to check the accuracy of your oven.  Adjust
your oven according to the manufacturers' directions.
Too much sugar in a recipe can also cause the cookie to brown too much.  
Make sure you measure accurately.

If your cookies are not browned enough

If your cookies are too crumbly
Did you thoroughly mix your dough?
Did you put too much sugar or shortening in?
Did you put enough egg in?

If your cookies are too tough
Did you use too much flour?
Did you use the right type of flour?
Did you use enough fat (shortening or butter)?
Did you re-roll the scraps too many times?

If your cookies spread too much
Your baking temperature could be too low.
Did you grease the pans too much?
Did you let the dough sit on warm pans before putting them in the oven?
Is there too much sugar, shortening or leavening (baking soda or baking
powder) in the dough?

If your cookies didn't spread enough
Is there enough sugar, shortening or leavening (baking soda or baking powder)
in the dough?
Is the baking temperature too high?
Did you grease the pans enough?

If your cookies stick to the pan
Did you grease the pans adequately or use parchment paper?
Is there too much sugar in the dough?
Did you let the cookies sit on the pan too long after removing it from the oven?
